<aside> ⚕️ ALL students (new and returning) need to go through the Sports Registration process with their parents/guardians before they can participate in athletics. All documents must be read and approved as this is mandated by state law.

[ ] Pre-participation Physical Exam: Physical exams are valid for 365 days from the date of the exam and must be current to the start of their next sport season. (The physical exam form is linked below and must completed by a New Jersey licensed physician who has completed the cardiac module needed in order to perform a sports physical.) Please submit only the medical eligibility form (last page) to the Rank One parent portal. The first 4 pages are to be kept on file with your child’s physician.
[ ] Health History Update Form: An updated Health History Form must be completed for EACH SEASON. This can be completed in the Rank One parent portal. Please note that this is a health history update on any changes SINCE THE LAST PHYSICAL EXAM. This is not a lifetime history.
[ ] Additional Waivers/Forms: Complete/sign annual forms electronically through Rank One.
[ ] Concussion Testing: All student-athletes are required to complete Baseline Concussion Testing. Click the link for more information.
[ ] Asthma & Allergies: If your child requires an inhaler for asthma or Epi-pen for allergies, please have your physician complete the action plans below and submit to the school nurse.

Rank One is our athlete eligibility and injury software. It will show the most up to date information on your child’s eligibility. It will also give you reminders if any of your forms are about to expire.

Any new student, transferring into W+H in the 10th, 11th or 12th grade, must contact Mr. Romeo at [email protected] so he can help you complete the required transfer form. Any student who attended a different high school in 9th, 10th or 11th grade must register as a transfer, regardless of whether or not they played a sport at that school. The good news is that most of our transfers are immediately eligible, but still need to file the form. Parents may also be required to complete an affidavit concerning your residency.
Any new student who is repeating a year of high school (if, for example, they attended 9th grade at their prior school, and are registering for 9th grade at W+H) must contact Mr. Romeo ASAP, as there may be some implications for their athletic eligibility in later years. (There is no need to report a repeat year from grades 1-8.)
